I finally got around to trying out the Mono Game version of Open Rails and found the results most impressive, especially when coupled with ReShade.
My question is as follows: How difficult is it to do a custom Mono Game build if you download the source?

done that myself, I'd say getting initial build running is not much more different than using the original Open Rails source. You may need some files/references updated for the first time.
I don't think there is an official repository for a MonoGame version, Carlo is providing patch files which could be applied to the original source. I've myself hosted changes (based on Carlos work) on GitHub and trying to keep in sync with the master source.
I'd recommend reading through the "Monogame masterthread" where most of the topic is covered.
