Is there any way to get better diesel engine smoke? All I get is some kind of square packets of black exhaust. Is there a setting or download from somewhere?

You're probably missing the dieselsmoke.ace image in the main texture folder. There's a replacement from 3DTrains that will probably fit the bill:

Hello, Mac.
The simplest way is to copy dieselsmoke.ace file from MSTS\GLOBAL\TEXTURES folder to same folder, within one of ORTS installaltion (where Route's You riding folder is stored)

Another one-as ErickC suggests-the patch by 3DTrains, so You'll have 3 variants of texture to choose (light, medium and heavy smoke): You have to rename copy of one of them to "dieselsmoke.ace" and compare, which one You like better (maybe, there are 3 *.bat-files for doing that for you included).

As that textures are white, You'll have possibility to define the colour of smoke (at stable and running-up states; it's pitty, that at running-down, the smoke turns white) by setting special ORTS parameters in ORTSdiesel section of *.eng -files of Your locos (See ORTS Manual.pdf, diesel smoke settings).
MSTS default smoke texture is black and not so transparent, so its "flexibility" is restricted using ORTS.
Yes, black squares is substitute texture, if dieselsmoke.ace is missing.
Another note: since You haven't dieselsmoke.ace, the overvead wires will be white, as MSTS used that texture for catenary too.
ORTS developers included special texture, "overheadwire.ace" to "Samples" folder of installation, so You'll have to copy that file to GLOBAL\TEXTURES folder too, if You don't like riding below white wires ;)

Hello.

Using the ORTSDieselEngines block provides additional options. You can even add a custom smoke level and amount of smoke per locomotive.
IdleExhaust (1.2) comment (idle smoke volume)
MaxExhaust (6.4) comment (maximum amount of smoke)
ExhaustDynamics (2.0) comment (transient at increasing engine [RPM] speed)
ExhaustDynamicsDown (0.8) comment (transient at declining [RPM] engine speed)
ExhaustColor (98291A0A) comment (smoke color)
ExhaustTransientColor (98291A0A) comment (smoke color at increasing engine speed [RPM])

ExhaustDynamics is described in the "8.2.1.1.7. Diesel Exhaust" section of this manual.
A detailed description of the ORTSDieselEngines block is provided in the "8.2.1.1.4. Determining the ORTS specific diesel engine" section of the manual.
