I was running an activity from Scottish Central Plus. I had saved the game a couple of times already.

After about 20 minutes of running I saved again and the game immediately exited back to the OR main screen but with 2 pop-ups showing the log file.

Looking at the log it is not obvious what happened - maybe 2 different threads were trying to write to the log at once?

Yes, it's as you say. Probably there was still a "hidden" Runactivity.exe running, which occupied for itself the OpenRailsLog.txt file.
Another possibility is that your antivirus is accessing such file.

Seems to be a one-off. I have re-run the same activity and saved multiple times without issues.
